38. Contusion of the eye

Blunt injuries of the first degree are characterized by reversible damage to the appendages and the anterior part of the eye, visual acuity and field of vision are completely restored.

With blunt injuries of the second degree, damage to the appendages, anterior and posterior parts of the eyeball is observed, mild residual effects are possible, visual acuity is restored to at least 0,5, the boundaries of the visual field can be narrowed by 10-20°.

With blunt injuries of the third degree, more pronounced residual effects are possible, a persistent drop in visual acuity within 0,40,05, narrowing of the visual field boundaries by more than 20 °.

Blunt injuries of the fourth degree are characterized by irreversible violations of the integrity of the membranes of the eye, hemophthalmos, damage to the optic nerve. Visual functions are almost completely lost.

Treatment. For intraocular hemorrhages, oral administration of rutin (vitamin P), ascorbic acid, and calcium chloride is indicated; in case of retinal concussion, dehydrating agents are prescribed (25% solution of magnesium sulfate intramuscularly, 40% solution of glucose intravenously, etc.), vitamin therapy (orally and parenterally, especially B vitamins). After a few days, oxygen injections under the conjunctiva of the eyeball, ionogalvanization with dionine or potassium iodide, and ultrasound are prescribed to resolve the hemorrhages.

With dislocation and subluxation of the lens, accompanied by constant eye irritation or hypertension, its prompt removal is indicated, with retinal detachment diathermo, cryo or photocoagulation with shortening (filling, depression, corrugation) of the sclera. With subconjunctival ruptures of the sclera, the wound is sutured, followed by conservative therapy with antibiotics and corticosteroids.

A metal magnetic foreign body is removed from the cornea with a magnet, they are removed using a spear-shaped needle, and disinfectants are prescribed (30% sulfacyl solution, 1% synthomycin emulsion).

Multiple small foreign bodies in the conjunctiva of the eyeball must be removed only if the eye is irritated. Large foreign bodies are removed with a lance-shaped needle in the same way as corneal foreign bodies, after instillation of a 1% dicaine solution.

Orbital injuries can be the result of blunt trauma and wounds. In some cases, they are accompanied by subcutaneous emphysema, which most often occurs when the lower inner wall is damaged and is characterized by crepitus, bone defects.

Fractures of the outer wall of the orbit may be accompanied by ruptures of the outer parts of the eyelids, hemosinus of the maxillary sinus, lockjaw. With fractures of the inner wall, various changes are observed in the lacrimal ducts, tears of the inner corner of the eyelids and severe damage to the eyeball. Injuries to the lower wall are complicated by hemosinus and fractures of the zygomatic bone.

Landing on the nucleus of a comet 21.10.2014

The European Space Agency has finally approved the date and place of the soft landing of the scientific module Phila on the surface of the nucleus of comet Churyumov-Gerasimenko. The spacecraft is currently located 10 km from the comet's nucleus. Such a small distance made it possible to carefully study its surface with all its features and evaluate the "pluses" and "minuses" of all possible places where a detachable module could be planted. As a result, the landing site, previously designated as option "J", was chosen, located on a smaller part of the comet's twin nucleus.

Rosetta will separate the science module at 09:35 CET (Central European Time) on November 12, 2014 at a distance of 22,5 km from the center of the comet. Landing should take place in approximately 7 hours, at 16:30 CET. Separation and landing signals for the Philae module should be received on Earth at 10 CET and 03 CET respectively.

"Now that we know where Rosetta should be targeting, we are one step closer to executing this exciting but risky operation," said Fred Jansen, one of ESA's mission leaders. "However, there is more a number of key procedures that must be followed before we can give the final command to "proceed to land".

In particular, on November 11, that is, even before the separation of Rosetta and the landing scientific module, it will be necessary to clarify the trajectory of the spacecraft and its orientation in space. And on the night of November 11-12, researchers should receive confirmation of the readiness of all systems of the orbital and landing modules for separation.

After Phil separates, the orbiter itself will perform an "up and away" maneuver to reorient itself and establish communication with the science module. The latter, during his 7-hour descent, must make an analysis of the composition of the ejected dust, gases and plasma directly near the comet's nucleus. He will also photograph the receding Rosetta, and the approaching surface of the comet's nucleus. After "landing", if everything goes according to plan, the Philae module will take panoramic pictures of the vicinity of the comet's surface. It is expected that the first images will be obtained on Earth within a few hours after the separation of the vehicles.

An hour after landing, Phil will begin scientific experiments that are expected to last 64 hours. In the long term, the study of the comet's nucleus will largely depend on whether the dust deposited on the solar panels interferes with their recharging. But in any case, by March 2015, as the comet approaches the Sun, the temperature inside the lander will become too high to continue work, and Phil's scientific mission will be completed.

The mission of the orbiter will continue for quite a long time. "Rosetta" should accompany the comet Churyumov-Gerasimenko until its closest approach to the Sun (passing the perihelion of the orbit of 1.29 AU in August 2015), and then, together with it, head back to the outer regions of the solar system. Of course, the spacecraft will not only fly around the celestial body, but also observe the processes occurring in the comet's nucleus, in its coma and in its immediate vicinity. The researchers hope that this mission will provide us with unique information about the early formation of our solar system, and perhaps even about the origin of life on Earth.
