Safe Operation

1. General requirements for labor protection

1.1. Persons (hereinafter referred to as nurses (washers) bath attendants) not younger than 18 years old are allowed to work in the bathrooms and baths for maintenance of the provided PNI,

1.1.1. passed obligatory preliminary (when applying for a job) and periodic (during employment - 1 time per year; on the basis of the center of occupational pathology - 1 time in 5 years) medical examinations (examinations) for recognition as fit for work.

1.1.2. past training:

  • introductory briefing on labor protection and introductory briefing on fire safety (when hiring);
  • initial briefing at the workplace with subsequent registration of work permits;
  • repeated briefings at the workplace (on labor protection and fire safety, sanitation) (at least once every 1 months);
  • target (when performing one-time work not related to his direct duties as a nurse (washer) bath attendant);
  • unscheduled
  • when working conditions change (the introduction of new or revised norms, rules, instructions in the institution; installation of new equipment);
  • upon appointment or transfer to another job, if new duties require additional knowledge of the rules and regulations;
  • in case of violation by employees of the requirements of regulations on labor protection, fire and environmental safety, electrical safety, safety precautions when working with the provided PNI, sanitation and occupational health, rules for providing first aid in case of accidents;
  • at the request of state supervision bodies;
  • according to the conclusion of the commissions investigating accidents with people;
  • when increasing knowledge to a higher electrical safety tolerance group;
  • when testing knowledge after receiving an unsatisfactory grade;
  • when there is a break in work in this position for more than 60 calendar days;

1.1.3. who have completed an internship at the workplace (from 2 to 14 shifts);

1.1.4. trained:

  • preliminary (when applying for a job within 1 month);
  • annual - on labor protection, fire safety, electrical safety, safety precautions when working with the provided PNI, rules for providing first aid in case of accidents at work, sanitation and occupational health.

1.1.5. passed the knowledge test:

  • primary (conducted by workers who first come to work);
  • regular (carried out at least once a year according to the program approved by the director of the PNI);
  • extraordinary (carried out regardless of the date of the previous inspection):
  • when working conditions change (the introduction of new or revised norms, rules, instructions in the institution; installation of new equipment);
  • upon appointment or transfer to another job, if new duties require additional knowledge of the rules and regulations;
  • in case of violation by employees of the requirements of regulations on labor protection, fire and environmental safety, electrical safety, safety precautions when working with the provided PNI, sanitation and occupational health, rules for providing first aid in case of accidents at work;
  • at the request of state supervision bodies;
  • according to the conclusion of the commissions investigating accidents with people;
  • when increasing knowledge to a higher electrical safety tolerance group;
  • when testing knowledge after receiving an unsatisfactory grade.

Workers combining professions must be trained in safe work practices and instructed in labor protection at all work performed.

1.1.6. In case of unsatisfactory assessment of knowledge of the "Rules ..." and instructions, repeated checks are scheduled no later than 1 month from the date of the last check. Personnel who showed unsatisfactory knowledge during the third test are not allowed to work in this specialty and must be transferred to another job.

1.2. The main dangerous and harmful production factors associated with the nature of the work of a nurse (washer) bath attendant are:

  • increased physical activity (including tension of the hands and fingers);
  • the possibility of infection with pathogens of various infections and their transmission to other persons.
  • toxic effects of various chemicals (disinfectants, detergents, etc.) when toxic substances are absorbed through the mucous membranes and skin, and enter the gastrointestinal tract;
  • the danger of causing physical injury from the provided;
  • psycho-emotional load;
  • increased temperature of water, equipment surfaces;
  • sharp edges, burrs and irregularities of washed surfaces, inventory, tools and fixtures;
  • elevated temperature, humidity and air mobility of the working area;
  • danger of electric shock;
  • insufficient illumination and lack of natural illumination of the working area.

1.3. Lifting and moving weights manually must be carried out in compliance with the following permissible loads:

1.3.1. Lifting and moving weights on a horizontal surface when alternating with other work (up to 2 times per hour):

  • for women - up to 10 kg (including when moving goods on trolleys (wheelchairs), the applied force should not exceed 10 kg.);
  • for men - up to 30 kg (including when moving goods on trolleys (wheelchairs), the applied force should not exceed 30 kg.).

1.3.2. Constantly during the work shift:

  • for women - 7 kg;
  • for men - 15 kg.

1.4. Women in connection with pregnancy are transferred to another job in accordance with the legislation of the Russian Federation.

1.5. A nurse (washer) bath attendant should use overalls and other personal protective equipment provided for by standard industry standards and standards for issuing sanitary clothing, shoes, etc.:

  • cotton robe or cotton suit - 4 pcs. for 24 months
  • cotton scarf - 4 pcs. for 24 months
  • towel - 4 pcs. for 24 months
  • brush for washing hands - on duty
  • waterproof apron - on duty
  • rubber boots - on duty
  • slippers - 1 pair for 12 months
  • rubber gloves - until worn out

When working with disinfectants:

  • rubber gloves - until worn out;
  • respirator on duty (PPE of the respiratory organs, which do not allow multiple use and are issued as "on duty", are issued in the form of a one-time set before starting work in an amount corresponding to the number of employees at a given workplace).

To protect hands when using disinfectants and detergents:

  • hydrophobic creams and ointments (100 gr. for 1 month);
  • regenerating cream (100 gr. for 1 month).

Washrooms should have enough toilet soap or liquid detergents for washing hands.

1.5.1. When involved in one-time work, the implementation of which provides for the provision of overalls, special footwear and other personal protective equipment (PPE), the nurse (washer) bath attendant is issued with the specified PPE for the duration of the work.

1.5.2. Sanitary-hygienic and overalls, sanitary-hygienic and special shoes, etc. PPE:

  • must be serviceable and subject to regular care (washing and / or repair, disinfection, disinfection in the PNI). It is not allowed to wash overalls at home;
  • those that fail prematurely must be replaced;
  • storage should be carried out separately from personal clothing, in individual two-section lockers.

1.5.3. In the event that an employee engaged in work with harmful and (or) dangerous working conditions, as well as with special temperature conditions or associated with pollution, is not provided with PPE, in accordance with the legislation of the Russian Federation, he has the right to refuse to perform work duties, and the employer does not have the right to demand from the employee of their execution and is obliged to pay the idle time that has arisen for this reason.

1.6. The nurse (washer) bath attendant must:

1.6.1. comply with the internal labor regulations in force in the PNI, work schedule, work and rest schedule, sanitary, safety and fire safety rules. Smoking is allowed only in specially established, marked and equipped places;

1.6.2. be able to provide first aid in case of accidents;

1.6.3. immediately notify your immediate or superior supervisor of any situation that threatens the life and health of people, of any accident that occurs at work, or of a deterioration in your health, including the appearance of an acute occupational disease (poisoning);

1.6.4. leave outerwear, shoes, headgear, personal items in the dressing room;

1.6.5. perform only the work that is entrusted to him by his immediate supervisor (head of the department, head nurse of the department, duty nurse of the department, in economic terms - the sister-mistress of the department).

It is not allowed to carry out orders and tasks that contradict the requirements of the Safety Rules.

1.6.6. follow the safety instructions when working with the provided APNI;

1.6.7. clearly know and comply with sanitary and hygienic requirements for the maintenance and cleaning of assigned premises;

1.6.8. observe the rules of personal hygiene;

1.6.9. take care of the skin of the hands, using protective and emollients, cut nails in a timely manner, clean the subungual spaces;

1.6.10. eat at designated places.

1.7. A nurse (washer) bath attendant may be suspended from work in the following cases:

  • ignorance and non-compliance with safety instructions, sanitation, fire safety;
  • appearing at the workplace in a sick state or in a state of alcoholic, narcotic, toxic intoxication;
  • in the absence of overalls and personal protective equipment necessary for the work.

1.8. In connection with the possibility of asthmatic manifestations and allergic reactions in the serviced provided, it is necessary to refrain from using perfumes, cologne, etc. at the workplace. funds.

1.9. The nurse (washer) bath attendant is responsible for improper performance or non-performance of her duties and violation of the requirements of labor protection instructions - within the limits determined by the current labor legislation of the Russian Federation.

2. Labor protection requirements before starting work

2.1. Put on clean special clothes (sanitary and hygienic clothes), safety shoes, pick up hair under a cap or scarf, remove earrings, rings. To perform various types of work, he uses appropriate sanitary clothing, overalls and safety shoes. You must follow the rules for the use of PPE:

2.1.2. do not pinch clothes with pins, needles, do not keep sharp, breakable objects in the pockets of clothes.

2.1.3. shoes must be made of non-woven material (available for disinfection), stable, with non-slip soles (it is forbidden to use shoes with high heels, with an open (non-fixed) heel).

2.1.4. it is prohibited for personnel to leave the department in special and sanitary clothing, shoes and other PPE intended only for work in the department.

2.1.5. PNI employees are prohibited from taking special and sanitary-hygienic clothing, footwear and other PPE outside the PNI territory at the end of the working day.

2.2. Before starting work, the nurse (washer) bath attendant should be familiarized by the nurse on duty with the peculiarities of the condition and behavior of the persons being served, and, in order to prevent cases of their aggression and auto-aggression, instructed on the necessary security measures.

Features of the mental state of those provided are recorded in the observation log, this log also records all cases of bodily injuries inflicted by sick personnel.

2.3. Check the workplace (bathroom, bathroom, etc.):

2.3.1. for the presence of objects with which the provided can injure the staff and themselves;

2.3.2. sanitary condition;

2.3.3. ventilation;

2.3.4. sufficiency of illumination;

2.3.5. check by external inspection the reliability of fastening, as well as the presence and integrity of the light-diffusing fittings of lamps (incandescent, fluorescent and quartz lamps). The use of lamps without light-scattering fittings is not allowed;

2.3.6. serviceability of electrical wiring, electrical equipment, sanitary equipment, mechanization and other items of workplace equipment;

Cases and covers of switches and sockets should not have cracks and chips, as well as bare contacts; switching and branching boxes must be closed with covers;

2.3.7. availability and compliance of cleaning equipment, detergents and disinfectants. Cleaning equipment (basins, buckets, brushes, etc.) must be marked (including by type of work), assigned to separate rooms, stored separately in closed sanitary rooms, cabinets or wall niches specially allocated for this purpose. Rags for wet cleaning should be clean, stored in the "clean rag" container, used - in the "dirty rag" container. For each type of treated surface, a separate rag is used;

2.3.8. serviceability of the cleaning tool:

  • handles of cleaning tools (shovels, mops, etc.) must be without sharp ends, sufficiently strong and smooth; buckets should be with well-fixed handles, without holes.
  • the absence of piercing and cutting objects in the cleaning material and rags for washing.

2.3.9. condition and serviceability of floors and other surfaces. If they contain dangerous and harmful substances (spilled fats, glass fragments, etc.), immediately remove them, observing safety measures;

2.3.10. before starting work on wiping the glass in the frames, the strength of the fastening of the glasses and the frames themselves should be checked;

2.3.11. serviceability of valves, taps and mixers for hot and cold water;

2.3.12. in the presence of tools and equipment: check the presence of guards for heated surfaces and moving (rotating) parts of tools and equipment;

2.3.13. check the compliance of escape routes with fire safety requirements: clutter and littering of premises, passages, the use of carpets on escape routes is not allowed.

2.4. Prepare a sufficient amount of disinfectants and detergents, washcloths, basins, rubber mats, according to the number of washable provided.

2.5. Report all detected malfunctions of inventory, equipment, electrical wiring and other malfunctions to your immediate supervisor and start work only after they have been eliminated. It is not allowed to make unauthorized repairs to the equipment.

3. Labor protection requirements during work

3.1. The nurse (washer) bath attendant bathes the patients in accordance with the approved schedule of the department and, for safety reasons, must constantly be in the bathroom, shower or bath near the patients who wash themselves (she washes the weakened and seriously ill herself) and monitor the behavior of the provided.

In case of excitation of the provided, report to the medical staff on duty and, together with other medical personnel of the PNI, take measures to ensure the safety of oneself and others (in accordance with the Instruction on labor protection when working with the provided).

3.2. During work, be attentive, not be distracted by extraneous matters, conversations.

3.3. Do not entrust your work to untrained and unauthorized persons.

It is forbidden to involve in the work provided without the permission of the attending physician (head of the department, psychiatrist). In the case of the use of patients in these works (in the order of occupational therapy), the administration of the PNI or department is obliged to take measures to ensure the safety of these works.

3.4. To prevent occupational diseases and comply with the rules of the sanitary-hygienic and anti-epidemic regime, the following work procedure must be observed:

1) treat hands under running water (using any available detergent) (soap - wash twice abundantly, others - according to the instructions on the bottle);

2) put on overalls, a waterproof apron marked "for cleaning", special shoes;

3) put on rubber gloves (household) and clean rags soaked in a disinfectant chemical detergent, treat the washing places (shower rooms, baths, baths, benches, basins, rubber mats, etc.) in accordance with the instructions; after washing, throw the rags into a container for dirty rags;

4) treat the apron, gloves and shoes in a disinfectant solution and then remove the apron and gloves;

5) wash your hands (see paragraph 1);

6) put on clean rubber gloves and a waterproof apron (marked "for the bath");

7) undress the person provided (or help to undress);

8) put the clothes of the provided person to be washed in special (rubber) bags marked "For dirty linen";

9) treat gloves (without removing them from hands) in a disinfectant solution;

10) together with the nurse of the department, conduct an examination of the department provided for the presence of pediculosis and the condition of the skin (including the condition of the nails);

The bathing of the beneficiaries who have these diseases is carried out last, after medical treatment and after instruction on special requirements for their washing;

11) in contact with infected skin, treat gloves in a disinfectant solution (without removing).

If gloves, apron or overalls and special footwear are contaminated with biological fluids, they must be treated in accordance with the disinfection treatment instructions and replaced with clean ones. Also treat contaminated surfaces in the room;

12) in the bathhouse, showers, ensure that the baths provided before entering and exiting the places of washing are treated in a special solution of the foot;

13) turn on a warm shower or take a warm bath (depending on the medical indications for this provided, but not higher than 37º C). Help the person being provided to get into the bath. Proceed to washing (use a specially processed washcloth, individual for each provided). In the bath, provided, able to wash themselves are given clean individual basins and a washcloth;

14) in patients with fungal diseases, at the end of washing, treat the lower limbs with a special solution (in accordance with the instructions). Processing of the bath and tools is carried out in accordance with the instructions for combating fungal diseases;

15) after washing provided, treat gloves and apron with a disinfectant solution, put the washcloth in a special container "Dirty washcloths" for further disinfection in accordance with the instructions;

16) seat the person provided on a special couch (bench) on a clean diaper and process (cut) the nails of the hands and feet. The treatment of cuts and other violations of the skin of the provided is carried out by a nurse who is present during the washing of the provided;

17) put the used towel and diaper into special bags marked "For dirty linen";

18) process gloves (without removing them from hands) in a disinfectant solution;

19) process the scissors in accordance with the instructions for disinfection;

20) dress the person being provided and transfer to the ward for escort to the junior nurse, ward nurse or other medical staff of the department;

21) treat the apron and gloves in a disinfectant solution (without removing them from the hands) and then remove them;

22) put on rubber gloves (household) and clean rags soaked in a disinfectant chemical detergent, treat the washing places (shower rooms, baths, baths, benches, basins, rubber mats, etc.) in accordance with the instructions; After washing, discard the rags in a container for dirty rags.

3.5. Before and after washing each provided person, treating his nails (haircuts, treating with special preparations), as well as after each contact with the secretions of the patient or contaminated linen, etc., after cleaning the premises, visiting the toilet and any contamination of the hands, and before eating, you must wash hands or disinfect them in accordance with the Instructions for Household and Hygienic Hand Treatment. The main way to disinfect hands is to wash them twice with warm running water using any available hand detergent (wash twice with plenty of soap, others - according to the instructions on the bottle). Particular attention is paid to the disinfection of the subungual spaces, the nails are cut short. Hands are wiped with an individual towel, changed daily and as it gets dirty.

3.6. In periods unfavorable for the incidence of influenza and other acute respiratory viral infections, wear four-layer gauze masks (or other special masks) that cover the mouth and nose. Masks are changed every 4 hours.

3.7. When working in the bathrooms of the isolator, you must:

  • when entering, put on a second gown and a special mask covering the mouth and nose, put on other safety shoes and PPE attached to the insulator (waterproof apron, boots, rubber gloves, etc.) and take them off when leaving;
  • After leaving, wash your hands.

3.8. When working with detergents and disinfectants:

  • use disinfectants and detergents authorized by the health authorities and approved for use in PNI in accordance with the instructions;
  • pregnant and lactating women, as well as persons who have not passed a medical examination or who have contraindications, are not allowed to work with disinfectants.

3.9. To prevent occupational diseases of the skin, eyes and upper respiratory tract when working with disinfectants, you should:

3.9.1. the preparation of disinfectant solutions should be carried out in separate isolated rooms with artificial or natural supply and exhaust ventilation;

3.9.2. pour dry disinfectants into special containers with the gradual addition of water;

3.9.3. do not exceed the specified concentration and temperature of cleaning solutions (above 50°C);

3.9.4. do not exceed the concentration of disinfectants;

3.9.5. Close containers with working disinfectant solutions tightly with lids;

3.9.6. store chlorine-containing solutions in a labeled container with a tightly closed lid (cork) in a specially designated place;

3.9.7. all work with solutions should be carried out with rubber gloves;

3.9.8. follow the instructions for use of the disinfectant;

3.9.9. use personal respiratory protection equipment specified in the instructions for the disinfectant (respirators, masks);

3.9.10. prevent spraying of detergents and disinfectants, contact of their solutions with the skin and mucous membranes;

3.9.11. when working with a chlorine solution, you should additionally wear glasses;

3.9.12. check the expiration dates of the multiple-use disinfectant solution, the possibility of its use in the future (no sediment, flakes, discoloration). On containers with ready-made solutions there should be a tag with the name of the drug, its concentration, date of preparation, use by expiration date (according to the instructions).

3.10. When performing work, it is necessary to comply with safety requirements:

3.10.1. open valves, taps on pipelines slowly, without jerks and great efforts. Do not use hammers, wrenches or other objects for this purpose.

3.10.2. filling the bucket, first you need to fill in cold and then hot water;

3.10.3. carry hot water for cleaning in a sealed container, and if a bucket without a lid is used for this purpose, then fill it no more than three-quarters of its capacity;

3.10.4. before washing the floors, remove traumatic objects: nails, broken glass, needles and other sharp (piercing and cutting) objects using a brush and a dustpan;

3.10.5. washing floors with rags using a mop; only washed rags are allowed to be squeezed out. Washed floors should be wiped dry;

3.10.6. when using water to remove dust from walls, windows and structures, electrical devices must be disconnected from the electrical network during cleaning;

3.10.7. when cleaning windows:

  • check the strength of the fastening of frames and glasses;
  • carry out work while standing on strong wide window sills, and if necessary (when working from a ladder at a height of more than 1,3 m) - using a safety belt and a safety rope, which with its free end must be fixed to the solid structures of the building. With narrow or fragile window sills, you should work from mobile tables - scaffolds or ladders - stepladders that have a platform with a fence.

3.11. When working with the use of ladders and ladders and scaffolding, the following requirements must be observed:

  • ladders must be tested, not have cracks in the bowstrings and steps;
  • to avoid slipping on smooth surfaces (tiles, concrete, etc.), shoes made of rubber or other non-slip material must be worn at the lower ends of the stairs;
  • sliding step-ladders must be with metal hinges in the upper part and devices (hooks, chains) that exclude the possibility of their spontaneous shift;
  • steps, rungs of wooden ladders must be cut into bowstrings, which are fastened with coupling bolts at least every 2 m;
  • it is forbidden to use stairs knocked down with nails without inserting the crossbar into the bowstrings;
  • the distance between the steps of the stairs - from 0,30 to 0,34 m, the distance from the first step to the installation level (floor, ceiling, etc.) - no more than 0,40 m;
  • the length of the landing ladder or ladder should not exceed 5 m;
  • admissible slope of ladders - 1:3 (60°);
  • permissible angle of inclination to the horizontal when installing a ladder - not less than 45° and not more than 60°;
  • you can not lift weights on ladders;
  • all ladders, ladders must have a tag with an inventory number and the date of the next test;
  • during operation, wooden stairs and ladders must be tested every six months, and metal ones - once a year;
  • it is forbidden to use untested stairs that are not on the balance sheet of the institution;
  • it is not allowed to perform work from ladders resting on window frames;
  • when working from ladders in the places of location of the provided, the movement of people to prevent its fall from accidental shocks, the place of installation of the ladders should be protected or guarded;
  • it is forbidden to use random objects (boxes, buckets, etc.) as means of scaffolding.

3.12. If in the course of work the workplace was contaminated with spilled solutions, shampoo, glass fragments, etc., stop work until the pollutants are removed:

  • remove grease spilled on the floor with rags or other grease-absorbing materials. The contaminated area should be washed with a heated solution of soda ash and wiped dry;
  • put the used rags in a metal container with a tight-fitting lid;
  • to remove spilled dusty powdery substances, wear goggles and a respirator. Carefully remove a small amount of them with a damp cloth or vacuum cleaner.

3.13. When using electrical equipment, various means of mechanization and devices, it is necessary to follow the rules (instructions) set forth in the technical passports attached to them and the instructions developed in the PNI.

3.13.1. connect electrical appliances to the electrical network with a flexible cable that should not be under your feet or touch metal, hot, wet objects (heating batteries, water pipes, etc.);

3.13.2. when turning off electrical appliances, pull the plug out of the socket by the cord, force must be applied to the plug body. Do not turn on or off electrical appliances with wet hands;

3.13.3. cleaning of places located in the immediate vicinity of electromechanical equipment should be carried out after a complete stop of the moving parts of the equipment;

3.13.4. it is possible to wipe electrical appliances (fans, electric fireplaces, electric radiators and others) if they are disconnected from the mains (the plug is disconnected from the socket);

3.13.5. the sockets located in the room, the switches should only be wiped with a dry cloth;

3.13.6. repair and cleaning of electrical equipment and electrical appliances belonging to the PNI is carried out only by the electrical personnel of the PNI, medical equipment - by a medical technician.

3.14. Work using electrical equipment must be stopped when:

  • the appearance of extraneous noise, the smell of burning, the interruption of the supply of electricity, in case of an accident of pipelines;
  • absence or malfunction of protective grounding;
  • fuzzy operation of the switch;
  • malfunctions of control and measuring devices, safety and blocking devices, fastening devices, fencing and other means of protection;
  • damage to the plug connection, cable insulation;
  • with open live parts;
  • the appearance of a tangible electric current on the equipment case;
  • breakage or cracks in the body of the machine (device).

3.13. Not allowed:

3.13.1. wrap the rings of scissors, handles of wire cutters, tweezers and other tools with cloth, insulating tape, and other materials to prevent skin and nail flakes from getting stuck in them;

3.13.2. catch falling scissors on the fly and walk with open scissors;

3.13.3. hang foreign objects (clothes, etc.) on switches or sockets;

3.13.4. store flammable substances outside the places specified in the PNI;

3.13.5. block lamps and other electric heaters with flammable materials (cloths, paper, etc.), keep cotton wool, alcohol and other flammable liquids near them;

3.13.6. use faulty equipment, home-made and personal electrical appliances that are not officially allowed for use in PNI;

3.13.7. dry napkins, towels, clothes, shoes, etc. on electric heaters (except for specially designed ones);

3.13.8. use open fire in the premises and on the territory of the PNI (except for approved special works);

3.13.9. leave electrical appliances on (including in standby mode) during long breaks or when leaving work (except for refrigerators and other continuous equipment);

3.13.10. leave medical, disinfectant and cleaning equipment in accessible places;

3.13.11. leave containers with hot water unattended;

3.13.12. sweep debris and waste into openings, hatches, etc. or thrown out of windows;

3.13.13. when collecting garbage - compact it in the urn (box, tank, etc.) directly with your hands;

3.13.14. put rags and any other items on the equipment;

3.13.15. touch with a rag or hands open and not protected current-carrying parts of the equipment, as well as bare and damaged wires;

3.13.16. to carry out wet cleaning of electrical wiring, electrical starting equipment, electric motors;

3.13.17. use faulty valves and taps;

3.13.18. use for cleaning water with a temperature above 50 ° C, as well as highly toxic and combustible substances (acids, solvents, caustic soda, gasoline, etc.);

3.13.19. wash hands in oil, gasoline, emulsions, kerosene;

3.13.20. wash and wipe windows in the presence of broken glass, fragile and faulty bindings or standing at the ebb of the window sill;
