Why did an American nuclear submarine sink right at the pier in 1969?

In 1969, the American nuclear submarine USS Guitarro (SSN-665) sank at a depth of 10 meters right at the pier. The accident occurred as a result of inconsistency between the actions of two groups of specialists: one of them filled the bow ballast cavities with water, and the other - the stern ones. Each of these standard operations was needed to calibrate the instruments, but the simultaneous execution of them by crews unaware of each other led to flooding. The operation to raise and restore the boat cost $20 million.
